Practice Nurse – 18 Hours per week

We are looking for a caring and enthusiastic Practice Nurse to join our friendly, supportive and patient-centred team for 18 hours per week due to our expanding patient population. The working days will be agreed with the successful candidate.

We are offering:

· Supportive, well-established clinical team of 6 GPs and experienced nursing colleagues

· Appointment slots tailored to patient need with protected administration time

· Strong Multidisciplinary Team Support

· Friendly and experienced administration team supports the delivery of high-quality patient care

The Practice is well supported by NHS Fife services, including:

· A Mental Health Nurse

· CTAC Services

· An In-Hours Urgent Care ANP

· Pharmacy Team

· District Nurses

Our multi-disciplinary approach enables us to deliver high quality, patient-centred care to our Patients and Community.

Key responsibilities for the position are:

· Managing Long-term Conditions including Diabetes, Asthma, COPD, Hypertension, CKD and CHD

· Keeping up to date with Chronic Disease Practices and Guidelines.

· Reviewing Chronic Disease Test Results (including bloods)

· Delivery of Lifestyle Advice and Health Promotion

· Ideally a Non-Medical Prescriber

· Attend and participate in Practice Meetings
