The role:
An exciting opportunity has arisen for enthusiastic and motivated Advanced Nurse Practitioners to join the developing Hospital at Home Service within Renfrewshire and East Renfrewshire HSCP. The post holders will work within a frailty-focused virtual ward model, delivering acute level care to patients in their own home or homely environment, in close collaboration with acute and community services.
You need to have a live NMC UK pin in order to be considered for this post.
All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.
Duration, Location, and Working Pattern:
- Location based in Royal Hospital Alexandra Hospital, Labs Building Room 20
- This will also include working within a developing virtual ward model (up to 18 beds).
- Permanent Contract
- Full-time position (36 hours per week)
- Shift pattern for this post are 8am to 8pm, 5 over 7 including public holidays
Key responsibilities:
You will work closely with acute front door teams, including the Emergency Department, Medical Assessment Unit, Acute Medical Unit, and Older Adults Assessment Unit to identify, assess, and admit suitable patients to the Hospital at Home virtual ward.
You will undertake advanced clinical assessment, diagnosis, and decision-making on a daily basis, working collaboratively with the responsible medical officer to agree and implement individualised care plans.
The role involves delivering hospital-level diagnostics and interventions within the patient’s home setting in line with established standard operating procedures.
You will provide ongoing clinical review and monitoring of patients within the virtual ward, contribute to MDT decision-making, and lead on discharge planning, ensuring safe and effective transition to community services when appropriate.
Knowledge, training, qualifications and/or experience required to do the job:
Essential
- Qualified Advanced Nurse Practitioner (ANP) – not suitable for ANP trainees.
- Advanced clinical assessment, diagnosis, and decision-making skills.
- Experience working at an advanced practice level within acute or community settings.
- Ability to work autonomously while contributing effectively to a multidisciplinary team.
- Knowledge of frailty pathways and hospital-at-home or virtual ward models (desirable).
- Ability to undertake hospital-level diagnostics and interventions in community settings.
- Strong communication and collaborative working skills.
- Full UK driving licence essential.
