An exciting opportunity has arisen for an enthusiastic and motivated registered nurse to join the Dementia and Frailty Service within NHS Grampian.

The successful candidate will work across GP practices, providing specialist assessment, support and intervention for older adults living with dementia, frailty and associated complex health needs. Working collaboratively with primary care teams, patients, families and carers, the post holder will contribute to the delivery of person-centred care aimed at improving outcomes, promoting independence and reducing unnecessary hospital admissions.

Although based primarily within GP surgeries, the post holder will be professionally supported and operationally managed through Secondary Care Mental Health Services, ensuring access to specialist knowledge, clinical supervision and ongoing professional development.

The role will involve:

  • Undertaking comprehensive assessments of patients with dementia and frailty.
  • Providing support, education and advice to patients, families, carers and healthcare professionals.
  • Working collaboratively with GPs, Practice Nurses, Allied Health Professionals, Social Work and wider multidisciplinary teams.
  • Contributing to anticipatory care planning and complex case management.
  • Supporting service development and quality improvement initiatives.

Applicants must be a first level registered nurse with current NMC registration and significant post-registration experience working with older adults, dementia care and/or frailty services. Excellent communication, assessment and clinical decision-making skills are essential, along with the ability to work autonomously and as part of a multidisciplinary team.

A qualification in Dementia Care, Older Adult Nursing, Mental Health Nursing or equivalent experience would be advantageous.

The successful candidate must be able to travel independently across the locality to meet the requirements of the post.
