Discharge to Assess/ Intermediate Care Team- Based in the Community
Are you an experienced Band 5/6 physiotherapist, with strong leadership qualities and assessment skills, seeking a new and diverse challenge?
West Lothian continues to invest in delivering community assessment and rehabilitation closer to home through our Home First programme. This programme has seen significant investment in our community AHP teams and has recently secured funding to introduce a community single point of contact to enhance the patient pathways.
The successful applicant would be joining a very stable and dynamic multi-disciplinary team with a passion for integrated work, putting patient outcomes and experience at the heart of their practice. Including working closely with our inpatient colleagues to identify and facilitate early supported discharge and provide rehabilitation in the community.
