Are you passionate about delivering outstanding care for older adults? We are looking for motivated and experienced nurses to join our innovative Acute Frailty Team.
As a Clinical Nurse Specialist, you will play a pivotal role in assessing, diagnosing and managing the care of frail older people across acute hospital services, working autonomously within a dynamic multidisciplinary team. This is an exciting opportunity to help shape and develop frailty services, improve patient outcomes and contribute to service innovation.

- Assessment Form - Within the application you are required to answer three questions giving you the opportunity to fully evidence why you are the ideal person for the job. Please ensure these statements are written by yourself and are unique to you. This section times out after 30 minutes.
