We are seeking to recruit a compassionate and motivated Community Mental Health Nurse to join the Older Adult Community Mental Health Team.

The successful candidate will work as a valued member of the multidisciplinary team, delivering high-quality, person-centred care to older adults experiencing functional mental illness, dementia and complex mental and physical health needs within community settings.

The role involves undertaking comprehensive mental health assessments, contributing to risk assessment and management, developing and reviewing care plans, and delivering evidence-based interventions that promote recovery, independence and wellbeing.
