An exciting opportunity has arisen for a 1st level Registered General Nurse to join the Care Home Team in supporting the care homes within Perth and Kinross Health and Social Care Partnership South Locality. We aim to build relationships and to deliver quality, timely and effective care and therefore the prevention of admission to hospital. Services continue to evolve to meet the changing demands and increasingly complex needs of an ageing population.
You will have the experience to carry out clinical assessments of care home residents, provide nursing care advice and escalate clinical concerns as well as providing education opportunities to optimise collaborative and patient centred working.
You will work in collaboration with the District Nursing teams, Advanced Nurse Practitioners (ANPs), Care Home Liaison RMN’s and the wider integrated Health and Social Care Teams.
You should have a genuine interest in working within an integrated team and the diversity it brings. We are looking for leadership qualities and excellent communication skills. You should be a team player who can also work autonomously with a wide range of clinical knowledge and skills and evidence of a continuous professional development.
You will be required to work flexibly across the locality to meet service needs.
Community experience would be an advantage.
You must be registered on Sub-part 1: RN1 or RNA or Sub-part 2: RN2 of the NMC Register

Hours of work are: 37.5 hours per week – job share considered (shift pattern 09:00-17:00 but potentially 08:00 – 22:00 within District Nursing roster to include weekends)
