As a Health care Support Worker, you will be working closely with primary care colleagues and Occupational Therapists currently developing roles within GP practices across South Ayrshire. This role will be particularly focussed on supporting patients with complexity, multi-morbidity and high levels of frailty. You will support Occupational assessment and intervention to maximise the prevention of deteriorating function pre-crisis. The key approach will include intervening early and without significant service intervention-including self-help management, patient led but with information, advice and signposting, slow down the process of frailty, reduce crises episodes, intervene timeously providing care in the right place, at the right time.
A car driver is essential due to the nature of the role.
Hours: 37.5 per week.
