     We have 3 posts fixed term for 12 months.


An exciting opportunity has arisen to join the Crisis Service.  The purpose of these posts is to promote joint working between inpatients and community settings.  It will ensure the transition from inpatient to home treatment is smooth, the communication up to date and accurate and the least restrictive option is provided at the earliest opportunity. This includes staffing resources as well as the effective management of supplies.  However, one of the core remits of this post would be to facilitate optimal use of available beds and home treatment. The Crisis/ Home Treatment Service In Reach worker holds a key role in the coordination of care, specifically in relation to timely discharge from mental health inpatient beds. You will be expected to be closely involved in discharge planning and immediate follow up from In-patient Acute Services.  It is essential that you are currently a Registered Mental Health Nurse/ Occupational Therapist or Social Worker and that you have professional knowledge acquired through degree supplemented by diploma level specialist training, experience, short courses.  You should have Specialist knowledge and application of mental health care.  The shift pattern for this post is Monday – Friday 9.00am – 5.00pm.  There are three posts available which will be based in North East, North West and South Glasgow.
