An exciting opportunity has arisen for two experienced healthcare professionals to join our forward thinking and dynamic team in the developing Perth and Kinross Hospital at Home/ Urgent Care Service. We as a service are looking for individuals who demonstrate a flexible approach to their work, and who have previous experience of working autonomously and within a team. You will need to demonstrate excellent verbal and non-verbal communication skills.
The Hospital at Home service is progressing as part of a Strategic National Project and will form part of our Integrated Community and Urgent Care Service response to the people of Perth and Kinross.
The ethos of this role is to deliver a person centred approach to support people to remain at home or in a homely setting. The multidisciplinary Hospital at Home approach provides rapid assessment and short-term treatment service with the dual aim of providing a level of acute hospital care in an individual’s own home that is equivalent to that provided within a hospital, consequently reducing pressure on acute inpatient services.
The post holder will be required to clinically recognise and respond to signs of acute deterioration in a patient’s condition, while carrying out clinical leadership. They should also have the ability to demonstrate strong Professional Leadership skills.
There will be a clear multidisciplinary team approach and close working with the Locality Integrated Teams across Perth and Kinross.
We are looking for an individual who has experience of clinical assessment and caring for people with complex care needs. You will have advanced verbal and non-verbal communication skills. The ability to work well under pressure and respond positively to unpredictable situations is essential.
We are committed to ensuring staff feel supported with their wellbeing and development through supervision, appraisal and training. Staff are expected to be part of a variety of Quality Improvement initiatives.
