We are looking for a creative and clinically excellent Nurse Consultant to help us drive our forward-thinking transformative plans for care for those with frailty within Dundee.
Working closely with colleagues in Dundee HSCP and NHS Tayside, you will be responsible for supporting and further developing our model of care for those with Frailty in Dundee. Alongside colleagues in Primary Care, Inpatient care and Community Care you will improve the health and wellbeing of people in Dundee, supporting them to live independently for as long as possible.
Along with fellow Nurse Consultants, you will provide professional and clinical leadership and support to nurses and advanced practitioners working in MFE/Frailty services in particular, and contribute to the development of wider urgent care services as part of the senior nursing team of the Health and Social Care Partnership. You will also provide direct senior clinical assessment and review for patients on the frailty pathway.
The successful applicant will be an advanced nurse practitioner with extensive clinical experience, expert knowledge and skills at a senior level in Frailty and/or Medicine for the Elderly, with excellent leadership and communication skills, evidence of positive partnership working and research ability.
Hours of work are: 37.5 (fulltime) Band: 8b
