SOCIAL WORK – SENIOR PRACTITIONER DISCHARGE HUB
COMMUNITY TEAM - INVERNESS
HIGHLAND COUNCIL BAND 10: £41,172 – £45,022 (pro rata) per annum
35 hours per week – Full Time, Permanent
This is a new and exciting opportunity to share your expertise as an experienced practitioner and support development of the social work team within a newly formed hospital discharge hub. The post will be community based but will reach into acute hospitals to facilitate the safe and timely discharge/transfer of patients. The postholder will work closely with colleagues of all disciplines from different areas within NHS Highland, across community and acute sites.
Working with the Social Work Team Manager, you will provide leadership, advice, support and guidance to social work staff, as well as those in the wider MDT when required, in order to meet professional standards and service objectives as set out within the terms of legislation, policy, guidance and delegated responsibilities.
You will contribute to practice, policy and procedure improvements and developments using your experience of day to day social work challenges, outcomes and solutions. The Senior Practitioner will have a central role to play in supporting team members through supervision and modelling good practice.
