Do you have experience of working in the field of mental health and have excellent communication skills and a flexible approach?  We are recruiting to two posts, one covering mainly the North of Angus and one mainly the South of Angus.  This is an exciting opportunity to be part of a new team comprising of a Social Worker and 2 Assistant Practitioners.  The team will provide a smooth discharge pathway for adults being discharged from hospital.  These posts will be part of the Community Mental Health Team but will spend a high proportion of time in inpatient wards, supporting the timely discharge of adults from hospital, the management of complex care needs post discharge and aiming to reduce readmission rates.

This role will cover the Angus Community Mental Health Teams and inpatients wards at Carseview Dundee, Murray Royal Hospital Perth and Dudhope Dundee. These new roles are part of the redesign of adult mental health services, and are additional to the current workforce.

We are looking for someone who can work autonomously, with support from the social worker and wider Community Mental Health Team as required.  The ability to travel across Tayside is essential. 

The successful applicant will be supported to develop their skills and maximise the opportunities to improve the health and social care outcomes for the population of Angus.

Based at either Gowanlea, Arbroath or Whitehills, Forfar, covering the Angus area.
