NHS Ayrshire and Arran provide a wide range of health and social care services, including Acute Hospital care & community nursing and social care services. We are the main provider of Health and Social care to approximately 371 000 people in the Ayrshire and Arran area.
We are seeking to recruit a number of motivated & dynamic Advanced Nurse &/or AHP Practitioners to join our newly established Hospital @ Home Service. This service will deliver hospital level care to the patient in their home setting or place of care, whilst under the care of a MDT including a named Hospital Consultant.
Candidates should be a registered nurse or AHP working within the professional standards held in the NMC Code or HCPC, ideally they would have an Advanced Practice & Independent Prescribing certification (although not essential).
Candidates must be driven to provide a person-centred care experience for individuals & have a thorough knowledge of frailty.
Candidates should have the knowledge & skills required as part of the MDT in managing the acutely unwell adult.
The successful candidates will work across professional boundaries & with both Health & social care services to achieve the best outcome for patients.
