Midlothian Community Respiratory Team, Intermediate Care

There are 2 full time permanent Band 6 posts available.
Midlothian Community Respiratory Team treats COPD patients in the community, preventing admissions, treating acute exacerbations in the patients’ home and optimising respiratory care in a holistic way. The team benefits from having a Psychologist and Dietitian.
We have a facilitated discharge pathway for patients with respiratory diseases other than COPD, where they can be facilitated home from hospital earlier and seen by the team until their acute problem resolves.
We have some exciting projects in Digital innovation happening in the team currently and are undertaking some other exciting service development projects.
We accept referrals from GPs, hospital staff, SAS and community health care professionals.
There is the opportunity in this post to supervise and teach Band 5 Physiotherapists, assistant practitioners, clinical support workers and students.
The Physiotherapist may develop and deliver training and education programmes for a wide range of professionals, patients, family members and carers.
