The Care Home Collaborative offers an exciting opportunity to work in partnership with Care Home and HSCP Teams across GGC to provide professional leadership to support, develop and drive improvements for both residents and staff within care homes and care settings.
This post is part of the Care Home Collaborative (CHC) which will provide integrated, collaborative and coordinated services for care home residents to contribute to improvements in quality of care standards as part of a coordinated multidisciplinary team approach. The Palliative Care Nurse will, as part of the Hub model, provide clinical expertise and support care to residents within care homes and care at home settings. The post holder will provide professional leadership, advice and direction to professional colleagues and team members.
The post holder, as an expert in all aspects of Palliative Care, will work in partnership with care home teams to identify training needs and provide access to education and training to support the application of evidence based best practice, as it relates to Palliative Care. The post holder will work collaboratively with multiple key stakeholders, providing both proactive and reactive support to care homes and hscp teams.
The post holder is accountable to the Care Home Collaborative Nurse Lead and is accountable for the delivery of agreed national and local targets and outcomes. The post holder is expected to fulfil this role within the parameters of established national and local priorities, policies and procedures. The post operates within the NHS Board’s own strategic framework, to which the post holder would contribute to the health care quality strategy ambitions of person centred, safe and effective care.
A key requirement of this post is to contribute to leading on the development of strategies that enable healthcare provision to meet the needs of the older people in accordance with national and local priorities. The post holder will lead on Quality Improvement methodology to develop standardized and measurable care delivery strategies as it relates to Palliative care. Develop and implement approaches to build capacity and capability in palliative care that supports residents to live their best lives aligned to what matters to them
The post holder promotes the scope of the service provided by the multidisciplinary team across the GG&C area, crossing both health and social care boundaries and in collaboration with the team in identifying, initiating and implementing audit and Quality Improvement projects which can extend to formal research projects.
