Community Link Worker – Annandale & Eskdale
30hrs per week, permanent contract, Band 3
Description of Role:
This is an exciting opportunity for an enthusiastic and highly motivated individual to join the Annandale & Eskdale Health Improvement team.
As a Community Link Worker you will work in the community providing 1-1 support to people to assist them in achieving their goals and aspirations. This could be supporting them with regard to readiness to change, motivation, improving health and social circumstances or self management of long term health conditions.
You will also work closely with GPs, Social Work, Nursing teams, and a range of other professionals and regularly attend multi-disciplinary meetings within the local Practices.
You will also have an active role to play in the new Home Team model, currently being developed.
Part of your role will be about engaging with partners in the Third sector and building a sound knowledge of the services and support that is available for people. You will also assist in identifying gaps in support and services and work with colleagues and the local communities to find solutions and co-produce services, initiatives or support to fill these gaps.

INTERNATIONAL NURSES
NHS Dumfries & Galloway can support OSCE Training for international nurses for vacancies based at Dumfries and Galloway Royal Infirmary or Mountainhall Treatment Centre Community Ward 1 for those who have completed CBT and English language. The following should be highlighted in your application: ~ Evidence of CBT completion date ~ Results of an English language test via IELTS or OET ~ Evidence of starting NMC registration If your application is progressed, documented evidence will be required.
