Background information on Flow Navigation Centre (FNC):
The national Redesign of Urgent Care (RUC) programme was launched on the 1st December 2020 following Cabinet approval and focuses on delivering an urgent care system that benefits the public; ensuring that patients are seen in the most appropriate clinical environment to minimise the risk of harm and ensure safety. A key driver is to ensure care is delivered as close to home as possible by minimising unnecessary face-to-face contact and maximising access to a senior clinical decision maker. The intended outcome of this is an overall reduction in self-presenters to Emergency Departments (ED) where care can be delivered more appropriately in another setting.
The new pathway for urgent care is for people to contact NHS 24 111 for triage prior to attending any acute site in order to ensure they receive the right care, at the right place, at the right time. After decision support and clinical supervision, NHS 24 direct calls to several outcomes, including to the Flow Navigation Centres (FNC) for minor injuries scheduling or for further consultation or scheduling to A&E for adults and paediatrics.
The aim of the FNC is to provide an alternative to hospital attendance or admission when suitable, or provide the ability to schedule care, removing delays for patients requiring further investigation and / or treatment. This could reduce clinical touchpoints, reduce conveyance to Emergency Departments and lead to the reduction of nosocomial infection. This may be achieved through direct telephone triage/near-me consultations with patients, or by providing a direct access route that SAS colleagues can utilise to allow professional-to-professional clinical decision support and advice.
Primarily, you will be a part of the specialised 24/7 team in the Flow Navigation Centre (FNC)– that aims to deliver the Right Care, Right Place model guided by the Scottish Government.
Patients are referred to the FNC via several routes including 111 and Scottish Ambulance Service. To optimise case decisions and reduce unnecessary Emergency Department presentations, the FNC team will use telephone or “Near Me” to conduct direct patient assessment before advising the necessary course of action. The FNC ANP is expected to autonomously take responsibility for advising self-care or redirection to alternative pathways.
The successful applicant will have current First level NMC registration, be a qualified and experienced Independent Prescriber (V300) with Advanced Clinical Examination Skills & History Taking, Pathophysiology module and will be able to demonstrate objective evidence of ability to perform autonomously at Masters Level (SCQF 11).
Applicants will also be considered credentialed with evidence of education and clinical experience of working autonomously at Masters Level.
In addition to the pre-requisite clinical qualifications for the particular role sought, the successful applicant must have a minimum of 5 years ANP/NP extensive clinical experience. We would expect the candidate to be able to demonstrate high standards of clinical care, be comfortable making sound autonomous decisions and to have excellent interpersonal skills. In addition, the role would include being part of the Acute medicine, Hospital at Night and ED team.
An attractive relocation allowance of up to £8,000 is available to successful candidates.
