SOCIAL WORK – TEAM MANAGER DISCHARGE HUB

COMMUNITY TEAM - INVERNESS

HIGHLAND COUNCIL BAND 10: £41,172 – £45,022 (pro rata) per annum

  35 hours per week – Full Time, Permanent

 

An exciting new opportunity has arisen for an experienced, enthusiastic and motivated Social Work professional to develop the new social work team within a hospital discharge hub. The post will be community based but will reach into acute hospitals to facilitate the safe and timely discharge/transfer of patients working alongside colleagues in the community districts and acute sites.


The successful candidate will provide leadership and guidance to social work staff as well as working closely with professionals from the wider multi-disciplinary team within the discharge hub and community services. The Team Manager will be solution focused as well as creative and passionate, drawing on considerable experience of social work practice and person centred outcomes.

 

You will contribute to practice, policy and procedure improvements and developments using your experience of day to day social work challenges, outcomes and solutions. 

Evidence of previous experience in a leadership role required.
