ICASS (Integrated Community Assessment & Support Service) aims o improve the quality of care and outcomes for older people in supporting them to remain independent within their own homes or communities.

It is intended that ICASS will provide a person centred and integrated focus on hospital at home, hospital admission / discharge, intermediate care and re-ablement and will provide seamless care for older people through closer alignment of service provision.  ​​​​​​​

There are 2 posts available:

- Permanent - 15 hours (Thursday and Friday; 9am – 5pm; there may be some flexibility on days worked)

- Fixed Term to cover maternity leave* - 15 hours (Mon / Tues / Wed; 9am – 2pm)​​​​​​​

These posts are part of the administration team, supporting clinical referrals to Hospital at Home and is the first point of contact to the Hospital at Home team and will be part of the team based at Adamson Hospital, Cupar.

These posts will play a critical role in the overall system operating 7 days per week. A high degree of accuracy is required to deal with the complex referral process.  

You should be educated to Higher, SVQ level 3 and / or have equivalent NHS experience.  A sound knowledge of Microsoft Office packages including Excel and a knowledge of databases is vital as you will be required to manage an electronic referral system and database. Excellent telephone and communication skills will be essential as well as the ability to work flexibly including some public holiday.  A positive outlook in relation to high quality customer care and team working is also required.
