An exciting opportunity has arisen within our South Care Home Liaison Team for a First Level Registered Mental Health Nurse with experience as a Band 5 and a keen enthusiasm to work in Older Adult Community Care within the Care Home Setting.
This role offers the post holder the opportunity to provide high quality care and support to our patients currently in Care Homes across the South of Glasgow. It will afford the opportunity to build working relationships with our Care Home colleagues to provide efficient and effective care to improve the mental health and well-being of our patients.
The Care Home Liaison Nurse is a key member of the Community Mental Health Team and is equipped with a range of knowledge and skills including the delivery of low level psychological therapies with particular emphasis on the delivery of the Stress and Distress in Dementia Model. Full training is provided for staff who have not undertaken this already and there is provision of regular Psychological Therapy supervision by the team Psychologist. There is further opportunity to engage in joint working processes with Social Work Services to ensure the best outcomes for our patients and to support the management of Person Centred Care. A high level working knowledge of anti-psychotic medication and subsequent monitoring procedures is required for this post as well as identifying means of supporting non-pharmaceutical methods of care in the Care Home setting.
This role also provides the post holder with educational responsibilities with an expectation that learning needs of Care Home staff are taken into consideration and incorporated into the care plan to ensure effective and safe patient care is delivered. The aim is to support the up-skilling of our Care Home colleagues with whom we work in partnership with to the benefit of the patients.
The post holder will be responsible for the management of a caseload and will be supported by the wider multi-disciplinary team. The scope of the role requires autonomous day to day decision making and skilled risk management
