This is an exciting opportunity for a pharmacist to join the Pharmacy Team and RES team in Renfrewshire HSCP for 12 months to cover a maternity leave.  

This exciting role will involve working with the Renfrewshire HSCP Rehabilitation & Enablement Service (RES), GPs, secondary care and primary care clinical teams to support and improve patient care.  This is a patient facing role and will involve taking on a prescribing role and developing your own clinical caseload in line with Achieving Excellence in Pharmaceutical Care objectives. The post is based within Renfrewshire HSCP and you would be expected to travel throughout Renfrewshire. 

Your role will be to work as a member of the Renfrewshire prescribing support team.  You will be based within the RES multidisciplinary team, and will work closely with RES colleagues, acute care colleagues, GPs, other practice staff, community pharmacists, care home staff, members of the primary healthcare team, and central team colleagues in supporting the development and application of high quality, safe, cost effective, rational and evidence based prescribing and management of medicines in the Health and Social Care Partnership (HSCP).   You will provide specialist pharmaceutical care to the full range of care groups across the HSCP, particularly patients referred to RES, which may include undertaking home visits to patients to address medication related issues, and working across traditional hospital / primary care boundaries, and the health and social care interface. You will provide education, advice and support to the multidisciplinary health and social care team on medicine related issues. 

You should have a sound clinical knowledge, good IT skills, excellent communication and interpersonal skills and be able to work autonomously as well as part of a team.  You must be registered with the General Pharmaceutical Council and be able to demonstrate a commitment to Continuing Professional Development.  You must have significant post registration experience.  You will be required to travel between hospital, GP practices, patient’s homes and care homes to carry out the duties of this post.  This post requires PVG Scheme Membership/Disclosure Scotland Check.
