Aberdeen City Health & Social Care Partnership is seeking dynamic and flexible individuals with a passion for supporting individuals in their own homes or a homely setting.
We are looking for:
Senior Healthcare Support Workers (Band 3) and
Assistant Community Practitioners (Band 4)
to join our Community Response Team.
This is a new team which has been set up to support the residents of Aberdeen City and our community services so that people are supported to remain in their own homes or a homely setting whenever possible. The team will have its own caseload of patients to support with a variety of different needs, but may also be called upon to support our Health & Social Care Partnership teams during periods of extreme pressure.
You will be given a robust period of induction in the following settings and there will be educational opportunities for you too:
Community Nursing Teams
Hospital at Home Team
Rosewell House Integrated Intermediate Care Facility
The ideal candidate will have experience of working in health and social care, will be a great team player and communicator with the ability to support people according to their changing health and wellbeing needs. You will be supervised and supported by senior staff, but you need the confidence to work alone and recognise your limitations, seeking guidance when required.
