This post is fixed term until 31/01/23 

The post holder will provide professional leadership and advice on all aspects of community nursing to staff within Older Peoples Services including community nursing (day & out of hours), treatment room, specialist nurses and practice development nurses.

The post holder will contribute to the effectiveness of the HSCP through ensuring nursing teams deliver effective, evidence based patient care and innovative clinical services.

There will be an emphasis on professional development which delivers clinically effective services in line with the clinical governance and organisational development strategies of the service. The post holder will participate in various forums and work closely with colleagues from other disciplines ensuring a muliti-agency approach.
Essential Criteria
    1st Level Registered Nurse
    First level degree  supplemented by specialist training or equivalent experience in area of responsibility
    Substantial clinical, operational and line management experience
    Experience of clinical leadership  at a senior nursing level together with a proven track record of leadership qualities
    Experience of change management and leading practice development initiatives.
    Experience of involvement in the setting, maintenance and monitoring of standards.
    Excellent communication, leadership and influencing skills.
    Be resilient and self motivated with the ability to work on own initiative. 
    Work autonomously whilst contributing effectively to team work at all levels.
Desirable
•    Project management experience
•    Current driving licence.

The shift pattern is; 5 days over 7 including Out of Hours
