As a Diabetes Clinical Nurse Specialist, you will be a registered nurse who has acquired significant Diabetes specialised knowledge & have current experience in the management of Diabetes clinical care. Preferably you will have previous experience in a Clinical Nurse Specialist capacity and have the knowledge and skill set to competently perform the necessary technical/invasive/physical aspects of care to support patient compliance with treatment & safe management of Diabetes.
You will act as a resource & provide expert advice and training for patients, facilitating an immediate point of contact for urgent patient consultation. You will plan efficient outpatient provision & independent follow up reviews to facilitate care, wellbeing & admission avoidance. Nurse specialist in-patient consultation/ reviews may also be required to meet service demand. You will be responsible for consultancy, teaching, audit, research, participate in service development in conjunction with the Diabetes Team /Team Lead and maintain Service objectives to meet the changing demands of delivering specialised Diabetes care across South / South West sector.
You must have the ability to work autonomously and possess a high level of interpersonal and confident communication skills to manage & support this specific client group You will be responsible to engage with various teams within Primary & Secondary care including hospital clinicians, Dieticians and General Practitioners & maintain excellent clinical links between the services, whilst upholding confidentiality at all times.
You will be expected to contribute professionally within groups which may cross-traditional or service boundaries and possess a high level of ITC skills including e- forms, Trak care and portal.
You will be based within the South Sector and shift pattern is variable Monday - Friday as the service requirements dictate.
It is essential that you are currently a NMC Registered Adult Nurse. This post requires membership of Disclosure Scotland's PVG Scheme.
