An exciting opportunity has arisen for a Band 6 Frailty Nurse within the Frailty Team, Perth Royal Infirmary. 


We are seeking to recruit a nurse with ambition and interest in frailty.  The Frailty Team is based in Ward 4, PRI and is actively providing a ‘Frailty at the Front Door’ model of care.  There is a strong focus on a multidisciplinary team approach and you will work closely with the medical team, advanced nurse practitioner, allied health professionals and also social work colleagues. 


Our service aim is to provide frail people a comprehensive geriatric assessment on admission to hospital and thereafter, create and deliver a person centred care plan to facilitate a safe and timely discharge. 


In this role you will need to demonstrate excellent communication and interpersonal skills to rapidly establish relationships with patients and their family’s to seek information and furthermore, to share discharge/transfer pathway plans.  Establishing and maintain links with community support teams and networking the professional teams throughout PRI will be required.


PRI provides commitment to ongoing learning and development and you will be given the opportunity and support to complete training in Frailty specific subjects.


You will be joining an experienced multi-disciplinary team and this post welcomes experienced staff nurses.


You must be registered on Sub-part 1: RN1 or RNA or Sub-part 2: RN2 of the NMC Register
