Band 4 HCSW – Community Stroke Team

An exciting opportunity has arisen within Clyde Community Stroke Team for an experienced HCSW to provide ongoing rehabilitation to patients at home, following their discharge from hospital. The ideal candidate will be an experienced, motivated and enthusiastic AHP HCSW who has experience of working within a rehabilitation setting.  

The post holder will be expected to work from the Community Stroke Team, based in the Royal Alexandra Hospital, Paisley. The Team facilitates timely discharge from hospital for patients following a stroke and provides continued rehabilitation of patients at home. The post holder will carry out specific tasks delegated by therapists to support AHP in providing this service.

Patient documentation and multi-disciplinary communication within the team is also a requirement of the role.

Regular supervision and support will be offered as well as training opportunities as required.
