Advanced Physiotherapy Practitioner for Cardiac Rehabilitation

Band 7

We are looking for a highly motivated and enuthsiastic person to join our team providing specialist care for cardiac patients.


This person plays a key role within our team and will work closely with the advanced nurse specialist, senior health care support worker and admin staff within the Heartbeat Centre.  The role includes utilising high levels of decision making and clinical judgement, ensuring that health needs are met through the disgnosis and the management of complex patient conditions including assessment, treatment and review planning process in partnership with patients and families.  The role is also to organise, provide and develop cardiac rehabilitation exercise/provision for suitable patients with Coronary Heart Disease (CHD), and to provide specialist advice about Cardiac Rehab (CR) to healthcare professionals.  The role encompasses that of expert clinician, educator and audit; that provides an education, advisory, support and resource service for the cardiac patients, carers, and health care professionals.
