This is a full-time permanent post.
The post holder will be part of the Care Home Liaison Nurse (CHLN) Service within the Care Home Nursing Team. The post holder will deliver integrated, collaborative and co-ordinated services to support nursing staff through expertise of caring for older people gained through experience within community, care homes, acute/ mental health in order to support high quality standards of care. The post holder will be managed within the Glasgow City HSCP Care Home Nursing Team and will be required to work in partnership with colleagues in both Primary and Secondary care and the independent sector as appropriate.
The post holder will provide an accessible, appropriate, high quality, culturally sensitive and effective liaison nursing service to patients residing in Care homes. This includes those being discharged from acute, tertiary care or transferring from the community to a care home setting. Working alongside and under the supervision of the Care Home Liaison Nurse in partnership with Care Home managers, staff and Practice Development Nurses the post holder will support General Practitioners and other colleagues in the ongoing provision of nursing services for residents within care homes.
Knowledge, training and experience required:
- 1st level Registered Nurse
- Evidence of continuous professional development
- Knowledge and understanding of frameworks to support chronic disease management
- Ability to work on own initiative
- Ability to work flexibly as part of a team
- Driving licence – desirable
