This is a full-time permanent post.

As part the Unscheduled Care Team within the whole system of Community Mental Health Services the post holder will work closely with all disciplines/agencies. The post
holder will under the direction of qualified staff to provide a service for individuals with complex mental health, physical and social care needs who present in crisis who may
require additional home treatment/care intervention over a 24hr period, also providing a specialist mental health service into the Mental Health Assessment Units, Emergency Departments and inpatients services within the General Hospital setting.
The post holder will be expected to take part in a wide range of duties as directed by a qualified member of staff, duties will include offering telephone triage and crisis
support to patients and carers experiencing mental health crisis , engaging with health, social care and other professional agencies to support the patient pathway during
unscheduled care.
The service provides assessment and access to mental health inpatient beds over a 24hrs period throughout GG&C. This includes arranging hospital admission for a patient should this be required.
Due to the rapid response nature of Unscheduled Care, the post-holder will be expected to manage a range of duties including participating in assessment procedures, accurate record keeping and participation in team meetings.
The service operates over a 24hr period and has a number of bases across NHS GG&C.
