An exciting opportunity has arisen within East Renfrewshire Health & Social Care Partnership (HSCP) based across the localities of Eastwood and Levern Valley.

We are looking to recruit a highly motivated and enthusiastic Band 6 Community Diabetes Specialist Nurse to join our Community Nursing Team. 

Working autonomously within the wider primary health care team and across the primary / secondary care interface in order to deliver highly specialised diabetes caseload management, treatment and care. The post holder will be responsible for making complex clinical decisions; including specialist assessment, care planning, implementation and evaluation of specialist diabetes plans of care, providing treatment and education to enable people with diabetes their families and carers to effectively self-manage their condition within the primary care setting.

The preferred candidate will have excellent knowledge and communication skills. Providing Specialist Diabetes advice and education to patients, relatives GP’s, District Nurses and other professionals within East Renfrewshire HSCP. 

The ideal candidate will be a registered nurse with a minimum of 3 years  experience at Band 5 and or above. Educated to degree level.  With a Specialist qualification or similar in Diabetes Care. Interested in the  management of and delivery of diabetes care within the Primary care setting.

A V300 prescribing qualification or working towards/willing to work towards is desirable for this post. A car owner/driver with full driving licence is essential
The shift pattern is 0830am-430pm 5 over 7 days
