An exciting opportunity has arisen for an Advanced Practitioner to join the Hospital Emergency Care Team to University Hospital Hairmyres
The Advanced Practitioners will work autonomously across Primary, Community and Acute Services during the in and out-of-hours period, covering seven days including public holidays. They will be responsible for the clinical decision-making for the population including the assessment and treatment of patients across the Primary, Community and Acute settings.
The Advanced Practitioner will work closely with the multidisciplinary team (MDT)/medical staff including the OOH Hub and Acute Services. Communicate on a daily basis about any patient they have seen overnight/ at weekend and who needs to be followed up by the GP / Acute Services during normal working hours
The Advanced Practitioner provides assessment, clinical care and management of both stable and acutely unwell patients and his/her family, making complex, autonomous decisions using advanced skills and knowledge, having overall responsibility for the assessment of care needs and the delivery of advanced care. Medical telehealth/telephone support will be available when required from an identified source
