We have an exciting opportunity for a Senior Case Manager to join our team at Salus.
The post holder will lead and manage the case management team and service provision as well as oversee the delivery of service from bases in Coatbridge & Hamilton. The purpose of the job is to develop the Salus case management services in line with government direction, promoting the biopsychosocial model of care to both workless and employed clients. They will develop these services in line with recommendations outlined in the ‘Black review’ of the health of the working population.
The post holder will ensure the team keep accurate records covering the subjects’ demographics, consent procedures, assessments and progress reports, intervention history and administrative and financial details of these interventions, and deliver these records in an agreed format timeously as stipulated in the protocols. They will provide a case management service for a specific client group including personally managing a caseload dependent on the needs of the service and will act as the client’s advocate at all times.
The following is required:
Knowledge and Experience
- Educated to/working towards/operating at masters level or equivalent experience* within a relevant field (social sciences, health sciences, psychology) with minimum of five years experience with a proven track record in the successful management of staff and other resources (eg senior charge nurse/team leader or equivalent).
- Further education/evidence of continuous professional development in area of speciality including study days/courses/post graduate qualification.
- Relevant experience, especially in working with vulnerable people.
- Excellent communication and motivating skills.
- Administrative knowledge and experience including excellent presentation and IT skills.
- Experience of working with people in the social work, community, primary care or hospital context is essential.
- Experience of audit and research.
- Awareness of political agenda around health and employability.
Analytical and Judgemental Skills
- Judgements involving highly complex facts or situations, which require the analysis, interpretation and comparison of a range of options.
- Advanced problem solving skills for interactions with clients and service providers.
Planning and Organisational Skills
- Planning and organising own and team members workload.
- Allocating and supervising work of team members.
- Development and compliance with Quality Management systems.
Physical skills
- Requires physical skills, which are normally obtained through practice over a period of time or during practical training (e.g. standard driving or keyboard skills).
*Please note the term ‘operating at’ masters level corresponds to possessing an equivalent level of knowledge gained through experience.
