Full Time – 37.5 hours per week / Consideration will be given to part time/job share
Fixed term for 12 months
Band 4 - Salary Range £23,709 - £25,982 per annum (pro rata if part-time)
Distant Island Allowance of £1,971 per annum (pro rata if part-time)
The post holder will provide a person-centred service that is responsive to the needs and interests of two GP practice populations in Shetland (Brae & Whalsay). You will support individuals to identify non-clinical issues affecting their ability to live well, and to identify personal outcomes and priorities that they would like to address. You will provide support to overcome barriers to addressing these and link them to local and national support services and activities. You will also build relationships and processes between the GP practice and community resources, statutory organisations, other health services and voluntary organisations. This is a hybrid role with some time spent working in the health centre, with the Health Improvement team and some remote working. As this is a pilot post you will also be involved in the evaluation of the project.
This contract is for a fixed-term 12 months as a pilot project. If you wish to apply for this post as a secondment opportunity, you will need to discuss with your line manager before submitting an application.
