This is a full-time permanent post. The hours or work for the service will be initially 10am till 6pm worked over Monday to Friday.
This post will be part Home First Response service and will be based within RAH. The Home First Response service provides a MDT approach to support patients identified as living with frailty to be discharged from hospital and works collaboratively across community teams to prevent admission.
The successful candidate will have experience within a care environment, have or SVQ level 3 or have equivalent be able to work effectively within a MDT team.
The post holder will work as part of a multi-disciplinary service, contributing to patient care, communicating and delivering delegated, condition related information to patients with complex and non-complex conditions as agreed with Home First Response Team. Delivering sensitive diagnostic, prognostic and therapeutic treatment to a designated patient/patient group. This post offers the opportunity to develop competencies across all health fields to be able to support nursing and AHP care plans, treatments and therapies as well as the opportunity to work closely with patients to enable them to live safely at home.
