DISCHARGE/FLOW MANAGER
Band 7: £40,872 - £47,846 pro rata per annum
37.5 hours per week – Full time, Permanent
An exciting opportunity has arisen for enthusiastic and motivated individuals with relevant health and social care experience to work as Flow co-ordinators in the Inverness District Based in the community, working directly with the integrated teams to proactively prevent avoidable admissions, facilitate patients being discharged from acute services/community hospitals into community.
We are looking for a suitably experienced people to provide co-ordinate and manage the flow of people through multiple Community Services. Ensuring that discharges from the services for individual patients are managed in an integrated way and provide a fast and efficient pathway for each individual patient. To assess and identify issues within the patient pathways, across all services.
You will be very organised and be familiar with using spreadsheets. The most challenging aspect of the role will be communicating widely with by NHS Highland Services and the Independent Sector. You must be adaptable, self-motivated and able to respond to the challenges that this role will provide. Relevant experience working within both the hospital and community environment would be advantageous.
