Health & Social Care Co-ordinator - Inverness District
Band 4 - £23,709 to £25,982 per annum
Permanent - 2 x 37.5 hours per week
A exciting opportunity has arisen within the Inverness District Integrated team for 2 Band 4 Health and Social Care Coordinator.
The HSCC plays an integral role within the Integrated Team gathering referral details from various sources including clients, their family members and other Health and Social Care Professionals. This information is then shared with the multi-disciplinary team in order to prioritise and allocate. The HSCC provides support to professional staff when dealing with urgent and critical cases in the community .
The successful candidate must have excellent communication skills, good time management and organisational skills and have a flexible approach to their work.
Full driving licence and use of car for work purposes is essential.
