About the role

We are currently recruiting for a Mental Health Services NurseTeam Leader Band 7, to join our  South Community Mental Health Team who have responsibility for delivering mental health treatment to the older adult population within South Glasgow  The CMHT works closely with inpatient wards : Balmore Ward an 18 bedded Dementia Assessment Ward and Banff Ward has 20 beds for Older Adults with functional mental health problems.  This south post will initially be based at Elderpark Clinic in Govan. 
The team are responsible for the community based care of older adults with a diagnosis of Dementia and who are perceived to have significant levels of distress associated with dementia. The psychological responses to stress and distress in dementia are assessed by the team and using a biopsychosocial model of care individuals and their families are supported at home.

The team also provides care for individuals  over 65 years old experiencing functional mental health problems.   Psychological and biological responses to these illnesses are assessed and monitored by the MDT and close links with the wider HSCP to ensure recovery focussed evidenced based coordinated care in the right place at the right time.

As well as nursing colleagues the team has 2 Consultant Psychiatrists covering various geographic catchment areas, dedicated Psychologist, Occupational Therapists and clinical support Workers who function as Multidisciplinary teams to ensure efficient and effective care provision.

The successful post holder will have professional and managerial responsibility for the CMHT including responsibility for facilitating and providing clinical leadership whilst ensuring a high standard of person centred care is delivered to patients and their families. 
This is a key leadership role in the delivery of safe, person centered, effective, timely and equitable care requiring the post holders to manage registered nursing staff and support staff  as well as  coordinating  input from  the wider multidisciplinary team.

As the an NTL you will be instrumental in the day to day running of the team leading on service improvement with a strong focus on training and education in your areas of expertise.
Reporting to the Service Manager for Older People and Primary Care you will also work closely with wider senior management within the Older People and Primary care Community services on delivery of clinical governance and service development processes including the meeting of key Community objectives. You will participate in multi-disciplinary decision making with regards to clinical and risk assessment/ risk management and plans of care. Monitoring nursing clinical activity/ workforce issues and ensuring resource efficiency is a critical aspects of the role.
We are seeking to recruit an individual who is enthusiastic about implementing change and who will support development of clinical practice whilst evidencing continuous improvement and learning in the delivery of safe, effective, and person centered services.

This role demands evidence of effective communication skills and the ability to work both independently and as part of a team. We are looking for an individual with enthusiasm and commitment who will enjoy the challenge the role offers. The Nurse Team Leader  will have responsibility for the delegated management of the Older People Community Mental Health Team 

The post holder will support Older people and Primary Care and Primary Care Service Manager in managing the effective deployment of the resources within the team and supporting functions such as absence management.  They will support effective communication both internally and externally to the team and also support integration between health and social care services.  

Responsibility for leading on clinical governance activity will lie with the post holder, which will include the setting and monitoring of standards of care and service, responding to complaints, staff support, career long learning, continuous development, clinical supervision, and the implementation of policy and clinical guidelines. For profession specific developments/supervision the post holder will link with the appropriate professional leads.

The post holder will also actively contribute to multi-disciplinary/agency clinical governance activity and may become involved in agreed pieces of work that are out-with their traditional sphere of practice. 

The post hold will carry a defined caseload as appropriate taking responsibility for the assessment, care planning, intervention and discharge. 

It is an essential requirement that you are a NMC Registered Mental Health Nurse and will bring to the role extensive post registration experience within a relevant area and/or demonstrate expert clinical and professional practice developed through experience. You will be experienced in leading teams and can work effectively as part of a multi-professional / multi-agency team. The NTL post holder will be required to work 0900-1700 Monday to Friday.
