As part of a multidisciplinary team the post holder will have responsibility for assessment of care needs, evaluation of care plans and ensuring the delivery of high quality care to residents.
The post holder will work closely with multidisciplinary team partners who contribute to the care of the residents in the care home these will include the GP; District Nurse Team; Dietician and OT amongst others.
The post holder will support and promote the ongoing development of care staff working in the care home environment enhancing the quality of care delivered in order to minimise risk.
The post holder will provide clinical advice and support and work in collaboration with multidisciplinary partners.
This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
