You will provide support and timely intervention to patients with chronic respiratory disease in the community. You will prevent unnecessary or unscheduled acute hospital admissions and provide specialist respiratory care to patients with COPD, including self-management. You will provide clinical expertise and support to healthcare colleagues working in respiratory care field. You will support the early discharge of COPD patients from hospital. You will link with the Respiratory Early Supported Discharge (ESD) team, and hospital based respiratory assessment services/respiratory nurse specialists. It is essential that you have a Degree or Diploma in Physiotherapy/Nursing and should be currently registered with the HCPC/NMC. You should have relevant post graduate experience including respiratory experience. The shift pattern for this post is Monday - Friday 8.30am -4.30pm .This post is for 37.5 hours. This post requires membership of Disclosure Scotland's PVG Scheme.
