Home First Response Service – Glasgow HSCP/ QEUH
Senior Pharmacy Technician, Frailty B5
1 Permanent post or 12 month secondment if preferred
To support the principles of providing care closer to home, and to reduce admissions to hospital a new hub and spoke frailty team at the front door of the QEUH has been developed known as the Home First Response Team. This is a multidisciplinary team that delivers interventions that enable patients to maximise their health and independence through a coordinated approach. The Team will act as a conduit between acute and primary care to ensure effective communication and care planning to optimise the patient pathway.
The broad aims of the service will be:-
• Professional assessment and support decision making around optimum place of care
• Service provision to address all levels of need, complexity and vulnerability
• Shift in service delivery towards local community provision.
• Development of patient centred pathways.
We have an exciting opportunity for an enthusiastic and self-motivated Pharmacy Technician to join the Home First response team.
As a key member of the team you will provide pharmaceutical care and advice to patients and the MDT. Although initially based at the front door of the QEUH this role will involve working across all sectors of care linking closely with acute, primary care and community pharmacy teams to improve medicines management, ensure seamless transition of care and deliver excellent patient care. This post will enable the successful candidate to develop their clinical knowledge in the new Frailty service.
This post requires excellent communication, negotiation, organisational and IT skills along with the ability to influence and affect change.
Minimum requirements
NC in Pharmaceutical Sciences or equivalent, SQV Level 3 or equivalent. Registered with the General Pharmaceutical Council
Minimum two years post registration
Accredited Final Accuracy Checking qualification
• SQA Pharmacy Dispensary Checking Technician qualification (PDCT)
• National Pharmacy Association Accuracy Checking qualification (NPA)
• Buttercups Training Accuracy Checking qualification
Experience of working in hospital pharmacy is desirable however full training will be provided to the successful candidate.
