New and innovative role within Moray Health and Social Care Partnership

Mental Health and Wellbeing Practitioners are provided in partnership between primary health care and mental health services for direct delivery of mental health access and wellbeing services in GP Practices for individuals with a range of mental health conditions. 

The role is to take a holistic approach to a person’s health and wellbeing, including education/health promotion, signposting and connecting them to appropriate community groups and to statutory services, with the aim of delivering both practical and emotional support. 

Mental Health and Wellbeing Practitioners will work as a key part of the primary health care multidisciplinary team, supporting the teams to strengthen community and personal resilience and reducing health and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity by increasing people’s active involvement within their local communities.

To provide a leadership role that prioritises own and others workload. To co-ordinate the workload off the band 5s and provide supervision to team members.

You will hold a current professional registration with either the Nursing and Midwifery Council, Scottish Social Services Council or the Health Professions Council, work across a number of GP practices within the Moray area and receive line management from Moray Primary Care Psychological Servives.
