The purpose of this post is to identify, assess and refer unpaid carers across acute and rehabilitation settings in Edinburgh and working closely with the 4 Locality Hubs. In particular, the post holder will be expected to support carers become more involved in discharge planning for patients they care for, and to ensure carers are aware how to access support in their own right once patients are discharged into the community.
There are two posts available; one will be based in the Western General Hospital and North West Locality, Edinburgh and the other in the Royal Infirmary of Edinburgh and South East Locality, Edinburgh.
This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
