The Community Nurse will work across NHS Lanarkshire to support the care delivery of those patients requiring nursing care delivery, assessment and treatment in all aspects of the community setting.

The post holder will work alongside the wider primary care team to support the delivery of care in relation to GMS sustainability and as part of the primary care improvement plan.

The post holder will work with other members of the primary care team to ensure safe and effective person-centred care.  This role will provide the post holder with the opportunity to work across the whole system supporting the primary care improvement plans.  The post holder will be a key member of locality teams, for example; work across the Community Treatment and Care and Urgent Care work streams to support care delivery.


It is essential that the successful candidate must be a car driver.
