The NHSGG& Clyde Cardiac Rehabilitation Service exists to deliver high quality person centre rehabilitation to cardiac patients and their carers, referred to the acute Division.
The key responsibilities of the service are to provide assessment, diagnosis and treatment to a diverse range of conditions and patient types, in a variety of environments. Therapeutic management utilises a range of physical, behavioural and cognitive modalities to regain function following surgery, illness, injury, dysfunction or disease. This includes the management of critically ill patients whose survival and recovery depends on appropriate physiotherapy intervention. There is also a key role in health promotion and self management. The post holder has a specialist knowledge and experience and contributes to the provision of multidisciplinary care and education to patients with cardiac illness. The post holder is responsible for a clinical caseload, providing specialist packages of care to patients and their families and communicates with the multi professional teams from both primary and secondary care. You will take a systematic and comprehensive patient history, determining the clinical cardiovascular status, risk factors and psychosocial needs of patients. Analyse and interpret the outcomes of investigations, taking into account a range of options and participate in the development of specialist programmes of nursing care for patients in order to facilitate their optimal and safe recovery. Collaborate with patients to plan, implement and evaluate specialist programmes of care for cardiac patients , utilising all available resources taking into consideration the lifestyle, gender and cultural background and ensure involvement with the patient, family, carers and significant others. It is essential that you are currently a Registered Nurse with the NMC and that you have significant post registration experience with at least 2 years nursing experience in Cardiology or Cardiac surgery. The shift pattern for this post is 8.00am to 4.00pm.
