Palliative Care Helpline Co-ordinator

End of Life Care Together is a partnership of Highland organisations with the aim of improving palliative and end of life care for people when it matters most.

An Innovative Approach

This is a novel collaboration with organisational lead from Highland Hospice, who are investing £1.5m in developing the project, and with NHS Highland providing strategic resources.

The initial three-year project is being carried out with embedded evaluation to demonstrate the value of the approach and a commitment across the partnership to take the learnings and establish a sustainable service going forward that improves equity and service provision for those in the last year of life across the whole region.

Key Aspects of the Service

The project has a multi-faceted, population approach to service improvement taking into account health, social care, voluntary, and community supports that enable optimal availability and access to community resources for those in the last year of life.

This involves:

• screening and better identification of those at risk of dying in the next year

• a single electronic plan of care with read and write accessibility across health and social care providers, and with patient / family access

• a coordinated 24/7 helpline accessible to patients, families, carers and professionals

• an increase in the provision of community care whether this be through structured statutory services or through community and voluntary supports

The aim is to simplify access to support while increasing the quality of community resource for this population.

The Opportunity

We’re looking to recruit an individual with significant experience working with health and care professionals.

Working closely and with support of the Service Lead and Senior Nurses in the Palliative Care Helpline you will work with hospice, acute, community and social care partners to develop a single point service that will meet their needs and those of the people they care for. The role will enable you to bring your knowledge and skills working with the rest of the experienced team.

You’ll be comfortable and experienced working in a project management structure, facilitating discussions and influencing change, as well as be at ease working in a dynamic environment without a clear plan laid out for you…supporting development of what the service looks like is part of the role.

Whilst your curiosity will help in the ideas you bring, you will also have an understanding of current Palliative and End of Life care strategies, developments and initiatives.

You will co-produce pathways required in each geographical area for referral to the coordination service in line with the EoLCT Partnership objectives, input to the development of the processes, systems and structure needed to provide the co-ordination and develop the service level framework that will enable the service to commence.
