Working within the North West Rehabilitation Service and sometimes including the intermediate care unit, this post offers an excellent opportunity to work within a multidisciplinary team providing assessment, treatment, intervention and advice to older and disabled adults with multiple pathology aiming for hospital admission avoidance and supporting hospital discharge. You will work autonomously in a diversity of health and social care settings taking on, in addition to the professional clinical role, case co-ordination, health promotion, audit projects, and the provision of expert advice on professional interventions.
You will manage a complex clinical caseload within the service, applying specialist knowledge and skills relating to their professional role and working as an effective and efficient partner in the interdisciplinary team . This is a community post and you will work in a variety of locations in the NW. You will be a HCPC registered occupational therapist. A current driving licence is essential. Experience of being part of a multi-disciplinary team and working in the community is desirable . The shift pattern for this post is weekdays and occasional weekends.
