Please note this is a fixed-term post for 9 Months. Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have a written agreement from their line manager to be released on a secondment before applying for the post.
We have the opportunity to be part of the community nurse team you will be responsible and accountable for the delivery of an accessible, appropriate, high quality and effective nursing service to all age groups in the community by working in partnership with patients, care providers and other agencies.
The post holder will ensure a person-centred approach to care delivery which is safe and effective to ensure and maintain quality strategy.
The post holder will function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency and experience to lead and enable the Community Nursing team to provide a quality nursing service.
The post holder will be a clinical expert, be involved in public health activities, be accountable for a caseload over a 24-hour period and be a vital member of the Integrated Primary Care team and other multi-disciplinary groups.
In addition, the post holder will be responsible and accountable for the leadership and management of the aligned community nursing team and other staff groups where applicable, linking into the management structure of the Locality.
KNOWLEDGE, TRAINING AND/ OR EXPERIENCE REQUIRED TO DO THE JOB
- Registered Nurse with valid NMC Registration
- SPQ qualification is not mandatory for this post
- Educated to/working towards/operating at Degree Level
- Post graduate expertise in leadership/ area of practice that demonstrates the required breadth of knowledge required to lead safety, effectively and efficiently.
- Comprehensive knowledge of clinical guidelines and standards within the speciality/area of practice.
- Specialist knowledge and experience of relevant conditions, pathology, policies and procedures associated with the speciality/area of practice ensuring that the level of expertise can be utilised to deliver leadership within the speciality/area of practice.
- Management qualification or equivalent gained through experience and continuous professional development
- Further education/evidence of Continuous Professional Development in the area of speciality including study days/courses/post-graduate qualification
- The post holder will possess excellent team-working/leadership skills and have the ability to motivate others and work using their own initiative
- Excellent listening, communication and interpersonal skills.
- Effective time management skills
- Working knowledge of basic information technology
- Support members of the nursing team to evidence competency and ongoing development in keeping with current pathway development.
- The Team Leader will assess, appraise and audit Nursing practice, with comprehensive knowledge of clinical guidelines and standards across community nursing
- Experience to lead, manage and enable the nursing team in the delivery of high-quality patient care to asylum seekers, refugees and displaced people across North and South Lanarkshire
- Be responsible for the effective and accurate use of team resources
- Support individuals in order to achieve optimum health and independence to integrate into the community
