We are seeking two motivated and confident team workers to join our integrated service as Health Care Support Workers.

The post is based within the Extended Community Care Team, Victoria Integrated Care Centre, Helensburgh and Lomond Locality.
Duties include provision of support to the wider community team in the delivery of person centred Reablement care within clients’ own homes. Key priority areas include; maximising independence and supporting individuals to remain safe within their own homes, facilitation of prompt discharge from hospital and actively working in a preventative way to reduce risk of re-admission.

The post holder will work alongside all health and social care agencies, providing intervention and support to individual clients in the Helensburgh and Lomond area.

Previous experience working in health and social care settings would be advantageous. Excellent communication skills, team working skills and the ability to work alone are essential.

You will need to be flexible and able to prioritise and manage your time effectively. Good IT skills are required.

You will be line managed by the Helensburgh and Lomond Occupational Therapy Team Lead.
