The post holder will act as an informed member of the diabetes team with specialist knowledge in diabetes care, support of diabetic clients and their families & carers. The post holder will be responsible for the assessment, planning, delivery and evaluation of high quality specialist nursing care programmes. He/she will be expected to work within appropriate guidelines and protocols, leading, supervising and deploying nursing staff. He/she will work as an autonomous practitioner whilst undertaking nurse led clinics
Community Diabetes Nurse, working as part of Community Diabetes Specialist Team, to support diabetes care management for people referred to the service with Inverclyde HSCP
